These terms and conditions ("Agreement") govern your access to and use of the QUESTQI.ca website ("Website") operated by [QUEST Program Administrators] ("we", "us", or "our"). 1. Purpose of the Website
The Website provides information about the QUEST Quality Improvement (QI) program, an initiative aimed at enhancing Type 2 diabetes care in Ontario. The Website serves as an entry point for physicians and healthcare providers to learn about the program and register interest in participating.

By registering on the Website, you express your interest in participating in the QUEST program. Registration does not constitute enrollment into the program. Once you register, you will be contacted with additional information and may be required to enter into a formal participation agreement. Participation is limited to licensed physicians and qualified healthcare providers in Ontario.
3. No Collection of Health Information
At this stage, the Website does not collect personal health information (PHI) or extract Electronic Medical Record (EMR) data. Any future collection of EMR data will be conducted through our third-party partner, Amplify Care, and will be subject to separate consent and compliance requirements under the Personal Health Information Protection Act (PHIPA).
4. Use of Information
Information you provide through the Website (e.g., name, clinic, email) will be used solely to assess your eligibility, initiate contact, and administer participation in the QUEST program. Your data will be stored securely and handled in accordance with applicable privacy legislation.
