Getting Started with QUEST Data Extraction
Step 1: Sign the Agreement
Once you’ve reviewed and signed the QUEST Data Extraction Agreement, you’ll receive a follow-up email from ICI Medical introducing you to the Amplify Care team.
Step 2: Schedule a Call with Amplify Care
Amplify Care will reach out to schedule a brief onboarding call. During this call, you’ll have the opportunity to ask questions, and the team will collect the necessary details to set up your EMR data queries. The date for the data extraction will be confirmed at this time, and system access will be arranged.
Step 3: Receive Your Data Report
Amplify Care will securely run the EMR queries and prepare your personalized data report. There’s nothing more you need to do—your results will be shared once available, with clear insights to support quality improvement in your practice.
Working in a Group Practice?
If multiple physicians at your site are participating, a single group call can be arranged. This often makes the process more efficient and ensures everyone receives the same information. Clinic administrators / managers are welcome to join the call, especially if they play a role in EMR access or coordination.
Amplify Care will provide IT support as needed.

Amplify Care searches are installed directly into the clinic’s EMR systems, such as TELUS PS Suite, OSCAR, and Accuro, allowing seamless integration within existing clinical workflows. To enable secure and comprehensive querying, the user account accessing the tool must be set up with administrator-level access.

Data extractions can be scheduled to run after clinic hours or during periods of low activity, ensuring there is no disruption to real-time EMR performance during busy times. This approach avoids interference with staff workflows and patient care, and often allows the process to run more efficiently, as more system resources are available.

The data elements to be extracted from EMRs for the QUEST program are limited to: Patient ID (used only to distinguish patients within the source system and not directly linked to external identities), diagnoses (T2D, CKD, eCVD), medications (SGLT2/GLP1), and laboratory results (eGFR, uACR, HbA1c).
Data extracted will be used to determine:
Total number of patients in your practice
% of patients with Type 2 Diabetes (T2D)
% of patients with T2D and established cardiovascular disease (eCVD)
% of patients with T2D + eCVD not prescribed an SGLT2 or GLP1
% of patients with T2D and chronic kidney disease (CKD)
% of patients with T2D + CKD not prescribed an SGLT2 or GLP1
% of patients with T2D missing eGFR and uACR in the past year

The data used in initiatives like QUEST is fully de-identified to ensure that patient privacy is protected. This means that all personally identifiable information, such as names, addresses, and health card numbers, is removed before any analysis is conducted. Only aggregate, de-identified data is used in reporting and evaluation, ensuring that individual patients cannot be identified at any stage of the process.

The extracted data is aligned with the Diabetes Canada Clinical Practice Guidelines, including the 2025 updates to pharmacotherapy and CKD chapters, ensuring that the analysis reflects current best practices. Reports are generated to highlight care gaps, identify strengths, and uncover opportunities for improvement, providing actionable insights to enhance the quality of diabetes management.

Each family doctor participating in QUEST will receive a customized report with actionable insights, empowering them to address specific areas for improvement in patient care. Once the entire cohort has been analyzed, a comprehensive report- including comparative data- will be shared. Importantly, the report is designed to maintain confidentiality, ensuring that no specific family doctor or clinic can be identified.
