Grounded in the College of Family Physicians of Canada cycle of assessment, reflection, action, and re-assessment, QUEST uses personalized EMR data to identify care gaps and support targeted improvements in practice. The program also assesses alignment with Diabetes Canada Pharmacotherapy and Chronic Kidney Disease (CKD) Guidelines, reinforcing evidence-based care.

First implemented in Ontario in 2025, QUEST has engaged over 80 family physicians and demonstrated improvements in diabetes management, EMR coding, and clinical processes. Refined through physician feedback and real-world use, QUEST is rigorously tested and proven in practice.

What Will QUEST Assess?
Together, these data points create a focused, guideline-aligned view of your diabetes population—linking diagnosis, risk stratification, monitoring, and treatment. This ensures that quality improvement efforts are not only data-driven, but also clinically meaningful, measurable, and directly tied to better patient outcomes.

Type 2 Diabetes (T2D)

T2D + established cardiovascular disease (eCVD)
T2D + eCVD treated with SGLT2 or GLP1

Data Privacy and Protection
The data used in QUEST is fully de-identified to ensure that patient privacy is protected.
This means that all personally identifiable information, such as names, addresses, and health card numbers, is removed before any analysis is conducted. Only aggregate, de-identified data is used in reporting and evaluation, ensuring that individual patients cannot be identified at any stage of the process.
