How to Register Your Clinic

Step 1: Download and Complete the Clinic Registration Template (.CSV)

• Click here to download the Clinic Registration Template.

• Use the provided template to list the name and contact information of any physicians in your practice who are interested in participating. You can include as many physicians as needed in one file.

• Save the completed file to your computer for upload in Step 3.

Step 2: Fill Out the Clinic Contact Information and Address

• In the form below, enter your clinic registration lead’s contact information and clinic address.

• The clinic registration lead is responsible only for submitting the registration.

• After registration, each listed physician will receive direct communication from our team.

Step 3: Upload Your File and Submit

• Use the "Upload Your Clinic File" button in the form to attach your completed .CSV template.

• Then, click “Submit Registration.”

• Each participating physician will receive a follow-up email with next steps.
